  1. I didn't get an open enrollment package, when will I get one?
    The open enrollment packages were mailed to everyone eligible to participate in open enrollment using the postal mailing address on file at USNH during the week of September 25, 2006. You have 2 options: you can log onto MyUSNHBenefits.net to access USNH's new online web site. You will need to enter your Social Security number and Personal ID Number. Your Personal ID number will be numeric, the first 2 digits of your birth month and date (mmdd). You can also contact or visit your campus Human Resources Office for assistance.

     

  2. What is my Personal ID Number (PIN)?
    In order to protect your security a Personal Identification Number (PIN) is required for access to the MyUSNHBenefits.net website. When you initially login you will use your birth month and date ~ mmdd. You will be required to change your PIN the first time you log in to MyUSNHBenefits.net.

  3. I lost/forgot my PIN-how can I get this information?
    A representative of MyUSNHBenefits.net at (800)978-1898 can give you your PIN. You may also contact the benefits administrator at your Campus Human Resources Office. They can give you your PIN once they have verified all required information.

  4. How do I update my PIN?
    You can update your PIN by accessing the MyUSNHBenefits.net website, log on and click on the option Change your PIN from top of the home page.

  5. Can you update my PIN for me over the phone?
    No, you must log on to the MyUSNHBenefits.net website and click on the option to Change your PIN.

  6. I only have one name, how do I fill in the first name field?
    All you need to do it put a period [.] in that space.

  7. How do I change my address?
    In order to make a change to your address, complete the change of address form available on MyUSNHBenefits.net under Benefit/Info Forms and forward it to your Campus Human Resources Office.

  8. Do I need to enroll online if I do not want to make any changes to my benefit elections?
    It is important that you go on line to verify and make corrections to any of the information relating to you, your spouse/same-sex domestic partner or dependents and to designate a beneficiary for your life insurance. This is the first time USNH is providing you direct access to your benefit information and we need your assistance to make sure all of the information is correct. You do need to go on line to enroll in an FSA for 2007 and to verify student eligibility for 2007 and to designate your beneficiaries.

  9. I am currently covering student age dependents on my medical and/or dental plans, what do I need to do to ensure that coverage is continued for them? I just sent information to CIGNA to certify that they were eligible for coverage.
    You do need to go on line and certify full-time students (unmarried full-time students ages 19 to 25) to continue to be eligible for coverage. The CIGNA confirmation certifies the dependent is still covered on your plan through 2006 but you MUST re-enroll during 2007 Open Enrollment and attach the dependent to the appropriate coverages, medical and/or dental. Certifying your student age dependents will now be completed through USNH's on-line benefit enrollment. It is a simplified and more efficient process. This information will be transmitted to USNH's medical and dental carriers electronically; eliminating the need for paper.

  10. Can I add or remove dependents from coverage? OR Can I add or drop coverage?
    You may add or remove dependents and enroll or waive coverage during the Annual Open Enrollment period, or when you experience a qualified change of status as described in the USNH policy available online at http://usnholpm.unh.edu/USY/V.Pers/A.5.htm (see USYV.A.5.2.2) Examples of a qualified change of status are marriage, divorce, birth or adoption of a child.

  11. Do I need to verify my student's full-time student status?
    Yes, In order to continue coverage for your dependent in full time student status (ages 19 - 25 and full-time student) you must verify your dependents each year. Log on to www.MyUSNHBenefits.net and proceed through enrollment, you will be instructed to click on the box that says full-time student and name the college that s/he is attending.

  12. I was making my 2007 elections and adding Dependents under the Dependent Section and I don't see a Stepchild designation. What should I do?
    When you get to add a dependent and complete the information, you will see beside relationship a drop down box. Within this box you will have a choice of child. A stepchild is designated as a 'child' when enrolling them in medical and/or dental coverage.

  13. What happens if I don't verify my dependent as a full-time student (ages 19 - 25)?
    If you do not verify your dependent as a full-time student, that dependent will not be enrolled in Medical and/or Dental after December 31, 2006.

    The benefits that you elected were for 2006.   15. What if I'm newly eligible for benefits during October through December?
    Please contact your Campus HR Office and they will help you enroll in your 2006 benefits.
    You will also need to elect benefits for 2007 if you would like a Flexible Spending Account (FSA) for Healthcare or Child/Elder care, you have dependents that are full-time students, or wish to change your Life Insurance beneficiary (ies).

  16. I don't understand what plans I am eligible for--can you explain the difference between the plans?
    A comparison of the medical plans as well as a schedule of benefits for each plan offered by Harvard Pilgrim Health can be accessed directly on Harvard Pilgrim's USNH specific website http://www.HarvardPilgrim.org/usnh. The USNH Human Resources Office has posted a Summary of Benefit Sheets for the Option A plans, dental plans, plan documents for life and long term disability as well as information about flexible spending accounts. These can be accessed online at http://www.usnhhr.unh.edu/open-enrollment.html or;
    You can call also call each medical and the dental vendor's customer service department directly:

  17. If I don't enroll by October 13th, can I enroll later?
    If you don't enroll before October 13th, you will receive a confirmation statement showing your current year benefit elections which will be carried forward for 2007, with the exception of Flexible Spending Accounts (FSA) for which an employee must make a 2007 election. If you have not verified student age dependents, coverage will end for them effective January 1, 2007. If there are errors for you to correct or if you want to elect a Flexible Spending Account (FSA) for Healthcare or Child/Elder daycare, or you have dependents that are full-time students you have until October 27th to make these changes online.
    Please note: Updates may be made at any time to your life insurance beneficiary (ies) by accessing MyUSNHBenefits.net and selecting the option "Beneficiaries."

  18. My spouse and I are both employed by USNH. How will our benefits work?
    If you and your spouse are both eligible for coverage as employees under the plan, only one will be eligible to enroll dependents. An employee cannot be covered as an employee and a dependent.

  19. Someone I work with has different rates than me - Why?
    Rates are based upon eligibility, work status and location. Plan Design and collective bargaining agreements factor into rate calculations. Please refer to your personalized Benefits Enrollment Worksheet to determine which plan options are available to you.
    Also note, your pay period deductions are based on your number of pay periods you have.

  20. I am a member of KSCEA one of the Keene Unions-- why are my benefits wrong?
    A tentative collective bargaining agreement has been reached and you will be notified of your updated benefits once a final agreement has been approved.

    These are the choices that are available to you as your current mailing address with zip code indicates that you reside outside of the Harvard Pilgrim service area.

  24. I am divorced and my court order directs me to provide Medical Insurance for my children, but they don't live with me, they live out of state. What do I do?
    You can provide coverage for yourself and your children but you can only elect one of our medical plans. Your options are to elect an HMO or Point of Service (POS) Plan which will only cover your children for emergency services. Or you may elect either the EBPA plan or the Preferred Provider Organization (PPO) plan which will provide additional coverage for your children as well as yourself. Since the PPO plan is not on your enrollment, please contact your campus Human Resources office for assistance.

  27. Do I have to elect a Primary Care Physician?
    If you enroll in the HMO or the POS plan, you need to have a PCP. If your current PCP is participating with Harvard Pilgrim Health Plan, your enrollment page will reflect as 'On File' If you are choosing a new provider or enrolling for the first time, you can either log on to MyUSNHBenefits.net and click on http://www.harvardpilgrim.org/usnh for a provider directory or click on the link to Harvard Pilgrim http://HarvardPilgrim.org/usnh which directs you to the provider page which allows you to select a provider. You will need to know the Harvard Pilgrim provider number to complete your online enrollment. If you do not elect a PCP, Harvard Pilgrim will assign you to a PCP however, you can change your PCP at anytime with Harvard Pilgrim.

  28. How long does it take to get my ID cards after I enroll?
    If you enroll by the Open Enrollment deadline of October 13, 2006, your new Harvard Pilgrim Identification cards will be mailed to your home address the week of December 15th and you should receive them before 1/1/2007. Please remember to let your providers and pharmacy know that you have a new medical plan with Harvard Pilgrim.

  29. What do I do if I need to see my doctor or need a prescription before getting ID cards?
    Contact your Campus Human Resources Benefits Administrator and they will contact Harvard Pilgrim. You will be enrolled within 24 hours from the time Harvard Pilgrim receives the request.

  30. What if I don't have the Social Security Number for a dependent that I'm trying to add?
    Although it is helpful to have the Social Security Number, you do not need to list the Social Security number for dependents.

  33. What is Life Insurance?
    USNH provides four (4) levels of group life insurance through MetLife to provide for a family as a result of an employee's death. USNH offers a $10,000 basic coverage plan (if you choose this option; you also receive an annual cash back amount) and a 1.5 times salary plan at no cost to the employee. USNH also offers a 3 and 4.5 times salary option of which the first 1.5 times salary coverage is paid by USNH. The employee annual rate is a calculation based on annual salary (regular budgeted earnings) and age In addition you are eligible for accidental death and dismemberment (AD&D) insurance which provides coverage at the same level you select for life insurance plus an additional $25,000.   34. Whom can I name as a beneficiary?
    You can name an individual, and/or a trust, and/or an organization as a beneficiary. You may also find the following helpful in completing your Beneficiary information.
    • Primary Beneficiary: Your primary beneficiary should be the individual(s) and/or a trust/ and/or an organization that you wish to receive the insurance proceeds. You may have the proceeds divided among several primary beneficiaries. Your total shares must equal 100%.
    • Contingent Beneficiary: Your contingent beneficiary should be the individual(s) and/or a trust, and/or an organization that you wish to receive the insurance proceeds if your primary beneficiary (ies) predecease(s) the insured. You may have the proceeds divided among several contingent beneficiaries. Once again your total shares must equal 100%.
    • Trust (ee) Designation: If you plan to have insurance proceeds distributed through a Trust, you will need the name, address and date of the trust. Your Trust (ee) will be held fully responsible for the application for and disposition of the insurance proceeds.

  35. How do I modify a life insurance beneficiary's record?
    When you go online and log on to MyUSNHBenefits.net you can change or modify your beneficiary information at any time.

     

  36. What is AD&D Insurance?
    USNH's Life and Accidental Death and Dismemberment Insurance provides an additional benefit if you are injured in an accident or for a covered loss as described in the plan documents. The plans are posted to the USNH Human Resources web site here .

  37. What is Long-Term Disability Insurance?
    USNH's Long Term Disability insurance provides an income benefit to you should you become totally disabled as approved by the insurance carrier. There are 3 options and the amount is based on the employees regular budgeted earnings; 50% (employee also receives an annual cash back amount) and 60% are fully paid by USNH. The 66 2/3% benefit has an employee contribution for 6 2/3% and USNH contributes the remaining 60%. The Plan Documents are posted to the USNH Human Resources web site here

  39. I want to confirm my web elections. Can I have a confirmation e-mailed to me?
    At the end of the online enrollment process, you will have 3 options to select from (1) You can print the Statement from your Browser, (2) email the Statement to yourself, or (3) request that a copy of the Confirmation Statement be mailed to your postal mailing address.

  40. I can't seem to print my confirmation statement, what's wrong?
    Your Pop-up blocker is most often the problem. How do I change the Pop-up Blocker settings?
    1. Open Internet Explorer.
    2. On the Tools menu
    3. Point to Pop-up Blocker
    4. Click Pop-up Blocker Settings.
    On some computers there will be an icon near the top of the screen that you can click on to turn off pop-up blocker settings.

  41. If I don't finish completing my annual open enrollment pages, what happens to my elections?
    If you exit out of the Annual Enrollment, your changes will NOT be saved. You will need to log back in during the open enrollment period. Open enrollment elections are not saved until you confirm your elections, check the authorization statement and request a confirmation statement.

  42. Why can't I enroll or change my Voluntary Benefits or Retirement now?
    USNH's annual open enrollment is for making changes to or electing core benefits for the upcoming year. Your core benefits are Medical, Dental, Life and AD&D Insurance, Long-Term Disability Insurance and Flexible Spending Accounts (Medical and/or Dependent/Elder Care). Contact your Campus HR Office to make changes to your Voluntary and/or Retirement benefits.

    You can enroll and/or make changes to your Retirement Elections after January 1, 2007. Log on to MyUSNHBenefits.net and select the option "Benefits." Elections become effective the first of the month following completion of all portions necessary.

  44. How/When can I enroll or make changes to Voluntary Benefits.
    You are only able to view your Voluntary Benefit elections through www.MyUSNHBenefits.net. You must contact your Campus Human Resources Office to enroll and/or make changes. You cannot currently enroll and/or make changes online.

  45. Why can't I change my PCP online when I am completing my online enrollment?
    You will be able to change your PCP online after January 1, 2007. You will need to log on to Http://www.harvardpilgrim.org/usnh and create an account with Harvard Pilgrim called HPHConnect (similar to mycigna.com). This will enable you to update your PCP information as well as use many other features.
    Please note: You may add a PCP at open enrollment online if you are adding a dependent for the first time
